Appointments
Our office schedules patient appointments Monday through Thursday from 7:30 a.m. to 11:30 p.m. and 1:00 p.m. to 3:30 p.m. If you are unable to keep your appointment, please notify our office 24 hours in advance. This will allow other patients to use that scheduled time. We ask that established patients bring your insurance cards and a list of your current medications at each visit. If you have not updated your information within the last 12 months, please complete a Patient Information Form and a Medical History Form and bring them with you to your appointment.

New Patients
On your first visit to our office, bring your insurance cards, drivers license, a list of all current medications, a list of any drug allergies, and your completed Patient Information Form, Authorization for Release of Information and Medical History Form.

Minor Consent
All minors (18 years and younger) MUST be accompanied by a Parent and/or Legal Guardian on their first visit to our office. The Legal Guardian must give consent and authorize all treatments. They must also give consent to use and disclose the Minor's health information for treatment, payment and health care operations. They must provide us with correct insurance information, as well as agree to pay all charges for any services performed by Cullman Dermatology Clinic, P.C. Please complete the Patient Information Form and the Authorization for Release of Information Form and bring it with you to your appointment.

Referrals
If a referral is required by your insurance company, it is your responsibility as the patient to ensure that you have a referral from your primary care physician. If you do not have the referral at the time of your appointment, your visit may be rescheduled or you may be financially responsible.

Payment
Cullman Dermatology Clinic, P.C. participates with a variety of insurance plans. Before making an appointment, please contact your insurance company to determine if our providers are contracted with your plan. If we are contracted with your insurance, we will collect any co-pay, coinsurance and/or deductible at the time of service. If we are not contracted with your insurance company, we require payment in full at the time of service. However, we will be happy to file the claim for you and then your insurance company should reimburse you directly. Services that are not covered by your insurance (e.g., cosmetic services) must be paid in full on the date of service. We accept cash, personal checks, debit cards, Visa, Mastercard and Discover. Our practice firmly believes that a good physician/patient relationship is based upon understanding and good communication.
