Mission Statement
Our goal is to provide the best possible dermatologic care individualized to each patient in a Christ-centered environment.


Appointments
If you are interested in scheduling an appointment, please call our office Monday through Thursday between 7:00 am and 4:00 pm. Feel free to ask any questions regarding our office policies. Please be advised that our staff is legally prohibited from providing any information about any medical condition if you have not yet been seen in our office. We also can not provide information about others without their expressed written consent. If, for any reason, you can not keep a scheduled appointment or will be delayed, please call us as soon as possible.

* Additional hours now available for Nurse Practitioner appointments - both cosmetic and medical. Call for availability. 


New Patients
On your first visit to our office, please bring your drivers license, insurance cards, and a list of all allergies and medications. To expedite your check-in process, prior to your appointment you can complete the required information using our Patient Portal. If you’re unable to use the Patient Portal, you may print the forms needed below and bring the completed forms with you to your appointment.

Established Patients
We ask that all patients bring their drivers license, insurance cards, and a current list of all allergies and medications to each visit. If you have not updated your information within the last 12 months, please use our Patient Portal prior to your appointment to expedite the check-in process. If you’re unable to use the Patient Portal, you may print the forms needed below and bring the completed forms with you to your appointment.

Minor Patients
All minors (18 years and younger) MUST be accompanied by a Parent and/or Legal Guardian on their first visit to our office. The Legal Guardian must give consent and authorize all treatments. They must also give consent to use and disclose the Minor’s health information for treatment, payment, and healthcare operations. They must provide us with the correct insurance information, as well as agree to pay all charges for any services performed by Cullman Dermatology Clinic, P.C.


Referrals
If a referral is required by your insurance company, it is your responsibility as the patient to ensure that you have a referral from your primary care physician. If you do not have the referral at the time of your appointment, your visit may be rescheduled or you may be financially responsible. 


Insurance and Billing
We accept most traditional insurance plans. Before making an appointment, please contact your insurance company to determine if our providers are contracted with your plan. If we are contracted with your insurance, we will collect any co-pay, coinsurance and/or deductible at the time of service. If we are not contracted with your insurance company, we require payment in full at the time of service. However, we will be happy to file the claim for you and then your insurance company should reimburse you directly.

If a service is not covered by your insurance, you will be informed of the cost and required to sign an Advanced Beneficiary Notification form prior to the procedure.  Payment in full is required on the date of service for any cosmetic procedure.
